Piping Failure Analysis
ASME Code for Pressure Piping B31.3 is an American
National Standard for Process Piping and contains
fiberglass installation and inspection criteria.
The subject photographs were taken during an inspection
of failed piping and fittings in a chlor-alkali
production facility located in Washington state. The
first photograph shows a saddle on a 6 inch pipe in
brine service that has been repaired and continues to
leak. The second photograph shows 2 inch piping and
elbows where excessive adhesive was applied, the fitting
“backed out” and leaked. These observed failures were
the result of faulty fitting workmanship training, not
adhering to practices specified by the pipe manufacturer
and ASME B31.3.
